***Sports Phyicals**
Any student who plans on playing a sport next season, must have a physical form completed and turned into Mr. Matchock by June 4. Physical forms can be obtained outside Room 208 and on Mr. Matchock's office door. Physcials will be conducted on Wednesday June 13 at 8:30am at the school. Physical form must be filled out and returned to Mr. Matchock prior to the physical. Click on the link below to obtain a copy of the physical form as well.
